We look forward to welcoming you to our office. To help ensure a smooth and efficient visit, please review the information below and bring the required items to your appointment.

What to Bring to Your Appointment

Please bring the following with you:

• Medical insurance card

• Previous X-rays or MRIs, if available
  (If your imaging was completed at Scottsdale Medical Imaging, we can access it electronically.)

• Referral from your primary care physician, if you have an HMO plan
  Please note: If we do not have a copy of your referral on file at the time of your visit, you may be financially responsible for the appointment.
  
  

Payment Information

Payment is due at the time of your visit. Please be prepared to pay:

• Co-payment (if applicable)

• Deductible and/or estimated patient responsibility (if applicable)

We accept Visa, MasterCard, American Express, Discover, cash, and checks.

Cancellation Policy

Please note that a $50 fee will be charged for same-day cancellations or no-show appointments.

Patients Without Insurance

If you do not have medical insurance, please inform our front desk when scheduling your appointment. Dr. DiNucci may offer special arrangements, depending on the urgency of your injury or condition.

Urgent Conditions

If you are experiencing a fracture, acute injury, or possible infection, please notify our staff when scheduling your appointment. We will make every effort to see you as quickly and efficiently as possible.
